The listed maternity-benefit documents include hospital and birth records, an NSSF card, an identity card, a claim form and a designated bank account. Those giving birth at a non-partner hospital must attend a postnatal check at an NSSF partner hospital.

The required documents include:

A hospital discharge letter or confirmation of childbirth
The child’s birth certificate or a copy of it
 An NSSF membership card
 A Cambodian national identity card
 A completed daily-allowance claim form
An ACLEDA Bank or Canadia Bank account
A letter of authorisation, if a relative is authorised to collect the payment on the claimant’s behalf, accompanied by that relative’s identity card.

If the birth takes place at a hospital that is not an NSSF partner, the claimant must attend a postnatal health check at an NSSF partner hospital to be eligible to apply for maternity benefits.
